For whom is this module suitable?

The module 'At home in your house' is an intervention intended for anyone who wishes to make an action plan to feel more at home in their house.

What is the content of the module?

The module was developed in close collaboration with an expert panel of practitioners and patients. It consists of the following three sessions:

  1. At home in your house: the patient reads information and tips to help them on their way. They determine whether their current home environment meets certain criteria and read about other people’s experiences. At the end of the session, the patient considers what they’re happy with, what they’re not happy with and what they’d like to change when it comes to their home.
  2. At home in your house: Make an action plan: the patient builds on the previous session by turning what they’d like to change into a concrete goal and action plan. They do so in five simple steps. At the end of the session, the patient is encouraged to implement their action plan.
  3. Take a look back: the patient considers where they stand and what they'd still like to work on.

Which diaries work well with this module?

The module can be combined with all generic diaries or with diaries that fit the goal of the client.

How do you use the module?

'At home in your house' can be used as either a complimentary module in addition to a (treatment) module or as a stand-alone module. The sessions are marked as guided, which means the practitioner provides online feedback after each session. However, as a practitioner you can choose to offer one, more or all sessions without guidance. This allows the patient to work through the module independently. The module is flexible in use: you can tailor it to your patient’s individual needs. For instance, by removing the ‘Take a look back’ session if you prefer to only evaluate the action plan in a face-to-face session. In general, the module can be used alongside face-to-face sessions as a way of continuing treatment outside of the therapy room. The answers patients give to the online exercises will enable you to prepare more specifically for future face-to-face sessions.
