Creating a relapse prevention plan

In the ‘Creating a relapse prevention plan’ module clients are guided in creating a relapse prevention plan. The answers from the module are saved in a separate plan that can be easily accessed from the homepage. This module is a brief version of the more elaborate module ‘Preventing relapse’.

Who is the module for?

The module can be used by clients who have a range of mental health issues and may benefit from a relapse prevention plan. In mental healthcare, various terms are used for a relapse prevention plan, including a signaling plan.

What is the content?

Those who follow the module will first map out their sources of stress. Then, the phases of the relepase prevention plan (green, orange and red) are introduced. Step by step, clients will think about their thoughts, behaviours and feelings in each phase. Additionally, clients will think about how they and their (social) environment can act appropriately in each phase. This increases their self-control and autonomy. A well-structured plan can also help shape how complaints are experienced.

The relapse prevention plan is a tool for preventing relapse. The answers clients provide in the module are collected in a separate plan, which can easily be accessed from the homepage and the app. This way, they always have it on hand when they need to view or update the plan.

How do you use the module?

This is a guided module. A therapist can assign the module to clients. For example, to prepare for a therapy session.

Clients use the module first, and later on the created plan.
